  • Your initial appointment will be your evaluation with the Physical Therapist (PT) and a Physical Therapist Assistant (PTA) in a private room to discuss your symptoms and medical history. The PT will then conduct a thorough hands-on evaluation and establish a plan of care individualized for you. Be prepared to undress enough that your therapist can access the body. You will be provided with cloth linens and a hospital gown for your comfort.

    After your initial evaluation, you will be put on a recurring schedule for an undetermined amount of time. We do this so you will have the same appointment for as long as it takes for you to reach your goals. During the course of your treatment, there will be re-evaluations completed to check your progress and change the plan of care as needed.

    Other questions specific to your plan of care will be answered during your evaluation.

  • Yes. Prior to your evaluation appointment you will receive an e-mail with a link to complete all of your paperwork. All of the paperwork is electronic, and no paper forms are available. Once you start the intake forms, you must complete it in 1 sitting. It takes approximately 30 minutes, so please be sure you have 30 minutes to dedicate to completing the forms prior to starting.

    If you are not able to complete your paperwork beforehand, you will need to arrive for your initial evaluation appointment 30 minutes early to complete the forms in our office.

  • Each appointment is 1 hour.

  • No, you do not need a referral in Illinois since it is a Direct Access state. However, we do request that you provide us with the name(s) of your physician(s) so we can keep them updated on your physical therapy.

  • No, we do not do cupping or needling as stand-alone services. These are techniques used in manual therapy that are done as part of a plan of care during physical therapy appointments.

  • Hands-on physical therapy is also known as manual physical therapy. This type of therapy involves hands-on, physical manipulation to evaluate, diagnose, and treat musculoskeletal pain. For more detailed information please click here.

  • We treat pain from head to toe. We offer specialized treatment for pelvic floor dysfunction and chronic pain. For more detailed information please click here.

  • Cancellations must be made at least 24 hours in advance to avoid a cancellation fee. Patients that are a no-show for their appointment will be charged a cancellation fee. We recognize that emergencies arise, so in certain circumstances the cancellation fee could be waived.
