Currently we only accept Blue Cross Blue Shield PPO Insurance.

If you do not have Blue Cross Blue Shield, we do offer a self-pay option. Please call our office to inquire about our rates. Self-pay amounts are due the day of the appointment.

We do not accept Medicare and cannot treat patients on Medicare for services that are covered by Medicare. However, Pelvic Floor Dysfunction (not including incontinence) is considered a non-covered service therefore we are able to treat patients with Pelvic Floor Dysfunction on a self-pay basis. Billing

Insurance plans vary and therefore it is the patient’s responsibility to know what their coverage is for physical therapy. It is also the patient’s responsibility to know if preauthorization is required. Please be sure to check your policy and/or call the customer service phone number on the back of your insurance card to find out what your physical therapy benefits are.

We will submit a claim to your insurance after your appointment. After your insurance accepts the claim, there may be a dollar amount remaining that is the patient responsibility depending on your deductible and out of pocket limits. You may pay your invoice by mailing in a payment, calling in a payment, paying at your next appointment, or paying online using the Pay Now button below.
